RADIOGRAPH and Ultrasonic REVIEW FORM
Objective
Compliance with:
ASME Section I (E2025) RT PW-51.4, PW-51.5; and ASME V Article 2, T-292; For UT PW-52.2
and A-302.11.
ASME VIII Div. 1 (E2025): RT (UW-51.a.1.(-a) & (-b), Appendix 10, 10-13(a); and ASME V
Article 2 T-292).
1. Ensure that the Manufacturer’s Quality Program incorporates the use of Review Forms
and NDE reports in accordance with:
a.- ASME VIII-1 for RT Review Form:
a.1.- App 10 (10-13(a)). The Manufacturer shall have a system for the
maintenance of Radiograph Review Forms and
a.2.- UW-51.a.1.(-a) & (-b), (1) A complete set of radiographic images and
Radiograph Review Forms, as described in Section V, Article 2, for each
vessel or vessel part shall be retained by the Manufacturer, as follows:
(-a) radiographic images until the Radiograph Review Form has
been accepted by the Inspector.
(-b) Radiograph Review Forms as required by Mandatory Appendix
10, 10-13.a.

b.- ASME VIII-1 for UT Review Form:
a.- Those does not address any retention or the preparations of UT Review Forms.
c.- ASME I for RT Review Form:
c.1.- PW-51.4: indicates that the Code Document is the RT Review Form and to
be as required by ASME V Article 2, T-292.
c.2.- PW-51.5: The Radiograph Review Form shall be retained by the
Manufacturer until the Data Report is signed and kept on file for a period of at
least 5 additional years.
d.- ASME I for UT Review Form:
d.1.- PW-52.2: indicates that the Code Document is the report, as required by
Section V, Article 4, T-493.
The report (UT Review Form) for each job shall be retained by the Manufacturer
until the Data Report is signed and kept on file for a period of at least 5 additional
years. A-302.11 refers to the reports.
